Promotional issue with obi, 6-page tri-fold insert, and bonus 7" single.Labels on both the LP and the bonus 7" single have 見本盤 (translation: "Sample Disc") printed on them. 7" single is taped to a cardboard insert.
Cover has extra wide spine to accomodate the LP, insert, and cardboard insert holding the bonus 7".
